The New Patient Referral Office is open Monday through Friday from 8AM to 5PM PST.
Physicians who wish to refer a patient to City of Hope may do so by also calling New Patient Services at 1-877-482-4673. You will be referred to the appropriate physician upon request for consultation.
To fax a request for more information, please print our online form. Fax the completed form to 1-626-301-8432.
